Guidance
Unacceptable if currently under investigation, or within 12 weeks of completion of treatment with tamoxifen or clomiphene.
Unacceptable if ever treated with pituitary-derived hormones.
Unacceptable if has received donated eggs or embryos since 1980.
Acceptable if treated entirely with non-pituitary derived hormones. If received urinary derived hormones, inform transplant centre.
